Inconsiderate people are everywhere including in church. It may not be appropriate for fellow church-goers to take immediately action. However I believe it is up to the church leaders including the presiding priest or pastor to remind members in the congregation not to persist in talking over the phone or allowing their children to misbehave.
My church takes very strong stand on this because the purpose of that event in question is for people to come into the fellowship of God, which by implication is a personal and holy experience. The understanding of the purpose of this event over-ride any sentiment of being patience with each other or to the young children.
I have two children who has learned to worship in silence. Never in anyone time of their growing up process were they allowed to behave indiscriminately in an occasion such as this.

I mean I can understand at 3 years old, the parents and grand parents are excited that their little grandson is enjoying himself. And though off tune, does make some musical notes as you said so yourself.

last week i was going up the escalator of my workplace building....then this boy in front of me start swinging a plastic bag he's holding......and hit my arm......the mother oso oblivious to it.......i feel like "kok-ing" the boy's head......but cannot lah.....not my son leh......

About the children thingy, I do not know that child you are talking about but let me share one incident concerning myself. I worship in a congregation full of children because many parents there. Once I was worshipping, this particular girl around 7 or 8 started dancing right in front of the worship leader and singing loudly. I was irritated of cos, its like hey this girl is so distracting, how can we worship God this way. But later on I found out that this little girl has Down's Syndrome and that she is like this, very active and very oblivion to people around her. But what struck me was her act of worship. She was oblivion to people around her, it doesnt matter that her dance was bad, her singing off key but she just sang and danced just like DAVID did!!! He danced shamelessly in front of everyone! It brought me to a realisation that sometimes we adults when worshipping God could not bring ourselves to abandon all and worship God but are always so aware of how people look at us. That little girl taught me a lesson in worshipping God with all your heart, all your strength, all your mind and all your soul.

I for one will not prevent my own children praising God in the top of their lungs during praise and worship, but I will ensure when the message is given or prayer is recited, they remain at the utmost best. As long as my children were old enough to read, they are to follow the ritual of worship in it's entirety when they are present during the service.
I think we are talking about ill-disciplined NORMAL children here.
As to the adults, I think that the trend of this discussion is not really about people who are genuinely worshipping God inspite of their out of tune singing. It is about people who do not turn off their mobile or put them at silent mode. It is also about people who carry on their personal conversation when it is time to worship. As someone in this thread had stated: why bother to come?
